PAACS Faculty Application

Apply to become a PAACS faculty member!

To apply as PAACS faculty, you must fill out the application below, and then upload the documents required for your application’s approval.

We also need recommendation forms (described within the application). Instructions for filling these out are at the bottom of the page.

Once everything has been submitted, your application will go through an approvals process with the PAACS Board of Directors. If you have any questions, please email terry.duffey@paacs.net

If you filled out the application on the old website, forms.paacs.net, you will also need to upload Additional Documents

Please have ALL of the following documents ready for upload when you begin the document upload process (max file size for each item total 128MB; jpg, gif, png, pdf, doc, and docx accepted):

  • Marriage License (if applicable) PAACS Graduates: only upload if you got married after initially entering the PAACS program;
  • Diplomas or certifications PAACS Graduates: upload all documents since having entered the PAACS program;
  • Letter from your pastor confirming:
    • You are an active member of your local church
    • You are in good standing;
  • Curriculum Vitae.

Additionally, if you are not a PAACS graduate, please be prepared to upload:

  • Birth Certificate;
  • A letter of recommendation from your church pastor or board;
  • A letter of evaluation of clinical skills from your chief of surgery or other supervising physician
  • A letter of evaluation of clinical skills from a peer;
  • A color photograph of yourself.

Recommendation Form

PAACS requests 3 letters of recommendation. These can be submitted via this online form:

Please note: If you use the online application, these recommendation requests will automatically be sent to the individuals you list within your application at the email you provide. There is no need to send the link to the recommenders unless you are applying via a different method.

They can also be submitted by downloading this document and sending it to terry.duffey@paacs.net ; please instruct recommenders to have the name of the faculty applicant in the subject line or body of email. Thank you!
