1. They don’t seem to actively pursue increasing their knowledge.

2. They seem to just meet the minimum requirements – an occasional CME or a Grand Rounds every few months.

3. Sometimes they seek out colleagues for more information. They pursue the occasional CME and come to Grand Rounds once a month or so.

4. I see them regularly at Grand Rounds and are not surprised when they reference a recent CME they have done.

5. They actively seek out opinions, experiences, and help from colleagues. You would be surprised if they weren’t at almost every Grand Rounds and CME you hear of.

How effectively does this faculty member teach during ward rounds, whether formal or informal?(Required)

1. They never seem to do teaching rounds – or if they do, they don’t seem to teach.

2. They occasionally (maybe 1-2 times a month) do some sort of teaching round. They don’t engage me when they do.

3. While infrequent, their teaching rounds are engaging. I look forward to our rounds together.

4. They do teaching rounds often, but don’t engage much beyond telling me what to write down, or the occasional differential diagnosis or management plan.

5. Every opportunity is an opportunity to teach with them, and they engage with me frequently during our rounds together, including leading discussions with me on pathology or management strategies.

How would you describe this faculty member's teaching and presence before, during, and after surgery?(Required)

1. If we talk, it’s because it’s required. I’m not sure if they don’t want to teach or if they don’t like me.

2. They involve me a bit in everything, and explain a little about what they’re doing or what they plan to do.

3. We discuss major cases thoroughly beforehand, and they let me do some cases myself; we discuss some cases post-op as well.

4. We have robust discussions on almost all cases beforehand. They let me do many cases, and will give feedback or demonstrations of new techniques when I do. They’ll give some feedback consistently after cases.

5. We discuss cases beforehand in detail, and I can tell they are progressively relinquishing the procedures to me, though they engage fully to supervise and take responsibility. They give solid evaluations after procedures and get my input on the case and post-op plan.
